Requirements to Join: Chiropractors

1. Current License in state in which provider is practicing.
2. Malpractice Insurance to the minimal level required by the state in which he/she practices.
3. Chiropractor is joining an existing CHA group practice or meets the Expansion Formulary requirements for member population in a specific county or zip code area.
