Requirements to Join: Behavioral Health Provider

CHA has set forth certain minimum criteria that must be met for a practitioner to become credentialed. Without thus a practitioner will not be considered for credentialing or recredentialing. The minimum criteria are as follows:

  1. Applicant is licensed to practice independently in all states for which contract shall apply. License is in good standing and free from restriction and/or without probationary status.
  2. Applicant graduated from an accredited professional school, and/or highest training program applicable to the academic degree, discipline, and licensure of the practitioner.
  3. Applicant has professional liability coverage at $1,000,000/$3,000,000 (or other level as required by law).
  4. Applicant must provider a current and complete CAQH application or profile summary.
  5. Applicant must report a work history, which at a minimum outlines the five (5) years immediately preceding the current date. This information must be reported on the provider's application, curriculum vitae or resume. Any gap greater than six (6) months must be explained in writing.
  6. Applicant is available for emergencies, or has established emergency coverage procedures.

CHA understands that licensing nomenclature and requirements vary from state to state. The following is a list of accepted Behavioral Health provider licenses for the states of Indiana and Michigan:

Indiana

  • Psychologist with Health Service Provider in Psychology endorsement
  • Clinical Social Worker
  • Marriage and Family Therapist
  • Mental Health Counselor

Michigan

  • Psychologist
  • Masters Social Worker with Clinical Specialty
  • Marriage and Family Therapist
  • Professional Counselor
