Affiliated Networks
In certain geographical areas of the state, such as Southwestern Michigan, Community Health Alliance contracts directly with other provider networks instead of contracting with individual providers. This type of agreement is an affiliated network arrangement with CHA.
If an Affiliated network is being utilized with Community Health Alliance, the member’s identification card and the payor’s EOB/EOP must identify the affiliated network with CHA. CHA will provide a block letter logo and the ID card logos must appear on the front of the card.
Examples of geographical areas where this information is required are:
- LCI- Lakeland Care Incorporated
This is the Michigan component of the CHA network and must also be on the ID cards and EOB in order to obtain provider/ hospital discounts.
- QCA-Quality Care Alliance
QCA-A Narrow Network that can use CHA as a tier two wrap.
- LPN-Lutheran Preferred Network
This is the Fort Wayne, IN area component of the CHA Network and must also be on the ID cards and EOB in order to obtain provider/hospital discounts.
