Welcome to Community Health Alliance’s Provider and Hospital Web-site.

Community Health Alliance is a network of carefully selected providers and hospitals working together in a cooperative effort to control the cost of healthcare. The network is accessed by employer groups as well as, third party administrators (TPA). When a member receives services from a preferred provider, their benefits are paid at a higher level. As you navigate within the web-site you will find a variety of tools such as, provider and facility requirements, service area, participating hospitals, contracts, policies and procedures, payor list, CHA newsletter and much more.
 
REMINDER: Please notify CHA of any physician terms immediately. Failure to report terminations to CHA can result in a delay in credentialing their new location(s), causing issues in claim payment & processing.
 
ATTENTION: Please Print new requirements for NP's, PA's, CNM's, and CRNA's.  Changes are effective immediately. 
 
 ** CHA Fee Schedules are updated annually for a January 1st effective date.  Additional codes can be added beginning in January for an effective date of January 1st.
