Community Health Alliance, a physician hospital organization (PHO), is a regional network of carefully selected physicians, hospitals, and other ancillary providers primarily operating in Indiana and Southwest Michigan who have joined forces in a cooperative effort to control the cost of health care, while assuring that quality services are provided.

CaregiverCommunity Health Alliance is a locally owned and operated partnership between Beacon Health System and Michiana Medical Associates, a physician group. The owners of Community Health Alliance believe that provider based managed care products will be able to compete effectively in the market for alternative delivery systems by meeting the market demands for quality, cost, size, and geographic distribution of the network providers and affiliated administrative service capabilities. Community Health Alliance, as a provider owned managed care organization, has an advantage over competitors due to the maintenance of local control and the ability to respond to customer demands in a timely manner.

Community Health Alliance offers freedom of choice in selecting a provider. At the time of service the member decides to use a provider in the network or can elect to go out of the network and pay more in out-of-pocket cost. Benefit enhancements are a part of the medical care plan to encourage members to stay within the network, thereby achieving a savings for the employee and the employer/payor.

The intent of this website is to provide you with the information necessary to carry out any administrative procedures required from Community Health Alliance. In addition, it will provide you with a comprehensive document outlining some of the most common policies and procedures of the network.
